Run! Day Camp for Dogs, LLC is committed to provide a safe and healthy environment for your dog to run, play, and stay while you are busy or away. To do that we ask that you complete this application.
We cannot enroll your dog at Run!, until this application and health forms are submitted, reviewed and approved.

BILLING AGREEMENT

I hereby authorize Run! Day Camp For Dogs LLC to run payments on the card information listed above. Payments will happen at the end of each week that I use the service in either day care or the overnight program.

Up-to-date vaccine certifications including: BORDATELLA – required to be administered at least once per year – recommended every six months. If the Bordatella vaccine is expired, or has never been administered, a 7-day waiting period is required after administration. DHPP – required to be administered at least once every three years – potentially every year depending on the dog’s age RABIES - required to be administered at least once per year Giardia AND parasite fecal screen results – (test must have been gien in the last 6 months) Leptospirosis – highly recommended, but not required.



PREVENTATIVE TREATMENTS

If your dog is discovered to have fleas or parasites, then, in our sole discretion, we may administer any flea, tick, or related treatment. If your dog is discovered to have intestinal parasites, then, in our sole discretion, we may place your dog in a private room. Or make arrangements for your dog to be picked up by you ahead of the original agreed length of stay. You agree to reimburse Run! for any related expenses.

TERMS AND CONDITIONS

Run! Day Camp For Dogs, LLC (“Run!”, “our” or “we”) asks that for the safety of your dog, our staff, and all others, to pleaseinitial after each section.

HOURS, SCHEDULING, AND CANCELLATION POLICY

Our office hours of operation are Monday through Friday 7am-5pm. All scheduling must be coordinated during normal business hours. For scheduling requests, please use our online request forms on the schedule page of our website.
Day Camp Service Policy: Any cancellation of Run! Day Camp services require 24 hours’ notice prior to the start of the requested service. You must receive a confirmation notification from us via email for the cancellation to be valid. If you do not cancel your appointment with 24 hours’ notice, then you will be charged for the full cost of the requested Camp Service.
Bunkhouse Service Policy: Any cancellation of Run! Bunkhouse services require a minimum of 72 hours notice. For peak holiday times, we require a full week's notice of any Bunkhouse cancellations made after the duration specified above, are subject to 50% payment of the reserved time.
Severe Weather Policy: Severe weather may require Run! to close for a period of time. We will notify you via our Facebook page and website by 6am if Run! will be closed for the day. Please check these sites daily during the winter months. Please be advised that the weather patterns in Vernonia may be different from those in Portland. Depending on the severity of the weather, we may limit pick-up only to designated “snow route” meeting locations, to be determined. It is the sole discretion of Run! to make a judgement call on weather conditions. Should foul weather, or the risk thereof, result in the Run! shuttle service being grounded and/or the Camp program being canceled and your dog is under the care of Run! at the Run! facility., you agree to the full charges incurred. This includes, but is not limited to, Bunkhouse, Camp and food charges. If the shuttle service is grounded, you are permitted to pick up/drop off your dog at the Run! property when schedules prior with Run! Run! is not required to transport your dog to/from Portland, should Run! deem the weather/road conditions are not permissible.

HEALTH AND SAFETY

You represent and warrant that any dog you enter into any Run! program is the following: at least four months old, if older than 7 months has been spayed or neutered, in general good health, has basic obedience training which includes the ability to respond to the basic call of “come”. Should my dog fail to come when called, I assume immediate responsibility for regaining possession of my dog. You represent and warrant that you do not have any knowledge that your dog has any dangerous or aggressive tendencies that could put other animals or Run! staff at risk. You acknowledge and agree that your dog will be playing off-leash, with a group of dogs. You agree it is your sole responsibility to maintain up to date vaccines and health requirements required by Run!. (DHPP, Rabies, Bordatella, monthly heartworm preventive or yearly heartworm test, monthly flea and tick preventative, parasite and glardia elisa fecal screens every 6 months) and that your dog is in good health.

VETERINARY SERVICES

You acknowledge and agree that if your dog is injured or becomes ill while at Run!, and, in our sole discretion requires immediate medical attention, then Run! may transport your dog and authorize any medical treatment recommended by Banks Vet Clinic. You agree to pay Banks Vet Clinic directly for all medical expenses incurred by Run! on behalf of your dog, even if such expenses were later found to be unnecessary.

PICK-UP AND DROP-OFF

Access to Residence: If you have arranged to have Run! pick up and drop off your dog at your residence or office, then you must provide Run! with the appropriate access. Our preferred method is a lock box with a code, garage codes, or coded entry systems. You acknowledge and agree that Run! may enter onto the designated property to pick up or drop off your dog, and you hereby release and agree to hold Run! harmless for any damages you or others may suffer as a result of Run! personnel entering the property to pick up or drop off your dog. If you provide Run! with key/s/ (One set for each Run! vehicle) Run! has a policy of not returning keys, and regularly purges its key collection for those clients that have not used Run! services in the last six months. If your keys are purged, then you may be required to provide Run! with additional copies of the key. You are solely responsible for ensuring that garage doors, key-coded entry pads, and any other electronic access device are operational and have sufficient battery life or electricity access.
Residential pick-Up/Drop-Off Times: The time of pick-up will range between 7am and 9am, and the time of drop-off will range from 3pm to 5pm. The time of pick-up and drop-off will likely vary during these time frames, as our transportation schedules vary daily.
Camp Shuttle Service To Your Residence: If we are unable to pick up or drop off your dog at your house, an additional charge of $20 applies to any alternative arrangements that need to be made due to lack of access to your home. In addition, there will also be a $20 fee for every ten-minute period that the Run! shuttle is held up waiting or making other arrangements for your dog. This fee will be assessed starting five minutes after our inability to access your residence, and will be assessed again after 15 minutes and after 25 minutes. After 5 minutes, we may leave your residence, and you will be charged for the full service requested, including all late fees. If you are unable to meet the shuttle at the one of the designated meeting times/locations determined by Run! you will be responsible for any additional transportation and overnight lodging fees.
Shuttle Service To A Designated Meeting Location/Time: If you do not arrive to pick up or drop off your dog at the designated shuttle meeting time/location as scheduled, an additional charge of $20 for every 10-minutes the Run! shuttle is waiting will be charged to you. This fee will be assessed starting five minutes after your meeting time, and will be assessed every five minutes thereafter. The Run! shuttle is entitled to leave the meeting location after a 5 minute wait. You will be charged for the full day of service, and any additional transportation. Late fees, and overnight lodging fees deemed becessary by Run!
Clean Up Fee: If your dog creates a mess of the Run! vehicles (poop, pee, vomit, or otherwise), then you agree to pay a $30 per occurrence van clean-up fee.
Collar and Leash Policy: All dogs being picked up by Run! must be wearing a properly fitting Martingale-style collar, with an identification tag. If your dog is not wearing a properly fitting collar, then we may provide the dog with a collar of our choosing at your expense. Run! also requires all dog(s) to be on a leash for pick up, and drop off. Run! will provide its own leashes used by our handlers. You are required to provide your own leash while handing your dog over to a Run! handler at pick up and drop off times. Similarly, during drop off, all dog(s) are to be on leash while in transit from the Run! van to your house, office or vehicle. This is for the safety of your dog, our employees, and third parties. For similar reasons, only Run! employees are allowed onboard the Run! vehicle.

BUNKHOUSE BOARDING

Medication: Prior to your dogs stay, you agree to email/notify Run! of a list of any medication(s) you are sending along with your dog(s). Any medication for your dog must be provided by you in a plastic, child-proof medicine bottle, which must be placed in a plastic zip-lock bag labeled with your dog’s name on the plastic bag. All administration instructions must be on the plastic, childproof medicine bottle. Medication presented to Run! that does not comply with the above directions will not be accepted by Run!. Please only send the amount of medication that will be required to be administered during your dogs stay. Run! may not return and may destroy any unused medication.
Pick-Up from Run! Facilities: If you choose to pick-up your dog from the Run! facilities, you must prearrange that pick-up. Any pick-up from the Run! facilities must be done before 10am, Monday-Saturday. Run! is closed for pick-up and drop-off on Sunday. The gates to the Run! facilities are locked at 10am each day, and a late charge of $30 will apply to any pick-up after 10am.
Personal Items: Aside from food and medication, Run! does not accept any personal items, including beds, blankets, toys, cups, or containers. Any personal items sent with your dog are not guaranteed to be returned, and Run! is not responsible for damage or loss of any items.
Food: All owners of dogs staying overnight are required to supply their own food. Food must be provided to Run! in a plastic sealable zip-lock bag, labeled with your dog’s name and feeding amounts on each bag. Any food placed in a different bag will be re-bagged and you will be charged a $5 re-bagging fee, per bag. If you do not supply any or enough food for your dog’s stay, then Run! may provide your dog with food, selected in our sole discretion, and you will be charged a $3 per meal food charge. You agree that Run! is not responsible for any allergic or similar reactions based on any food that we may provide. Run! does not return any unused food, so please only provide enough food for the length of your dogs stay. If you reside outside of our pick up area, we will arrange to have you meet the Run! van at a designated time and location. There will be a $20 late fee for every five-minute period that you are late in meeting the Run! van. This fee will be assessed starting five minutes after your meeting time. After 10 minutes, we may leave the meet-up location, and you will be charged for the full service required, including all late fees, additional transportation costs, and overnight fees.

PAYMENT TERMS

You agree to maintain a current, valid credit card with Run! You agree that Run! may charge any fees or costs assessed to you by Run! to your credit card on file. If your credit card is declined for any reason, then you agree that Run! may pass any additional charge assessed by any financial institution directly on to your account.

LIABILITY RELEASE; INDEMNITY

In consideration for allowing your dog to participate in Run! programs, you hereby release from liability and waive any right to sue Run!, its employees, managers, and agents from any and all claims, including negligence resulting in any physical injury, infestation, illness (including death), or economic loss you, or your dog, may suffer or which may result from my dog’s participation in Run! programs, or your use if the Run! facilities. YOU DO HEREBY KNOWINGLY, INTENTIONALLY, AND IRREVOCABLY WAIVE ALL CLAIMS WITH RESPECT TO THE FOREGOING, WHETHER BASED IN CONTRACT, TORT (INCLUDING NEGLIGENCE), STATUTORY, OR OTHER GROUNDS, EVEN IF ANYONE HAS BEEN ADVISED OF THE POSSIBILITY OF SUCH DAMAGES. OUR TOTAL LIABILITY FOR ALL CONTRACT, TORT (INCLUDING NEGLIGENCE), STATURORY, OR OTHER CLAIMS ARISING OUT OF OR RELATING TO THIS AGREEMENT OR YOUR USE OF RUN! SERVICES IS LIMITED TO THE LESSER OF THE VALUE OF ALL SERVICES PROVIDED TO YOU WITHIN THE LAST THREE MONTHS OR $100. SOME JURISDICTION DO NOT ALLOW FOR ANY LIABILITY LIMITATION; AND, IF SUCH LAWS APPLY TO YOU, THEN SOME OR ALL OF THE ABOVE DISCLAIMERS, EXCLUSIONS, OR LIMITATIONS MIGHT NOT APPLY TO YOU AND YOU MIGHT HAVE ADDITIONAL RIGHTS. You hereby agree to indemnify and hold harmless Run!, its employees, managers, and agents from and against all claims arising out of or relating to your dog’s participation in any Run! Program, or your use of the Run! facilities.

GENERAL

You hereby represent and warrant that all information supplied by you in you application is true and complete, and you hereby agree to promptly update any information when needed.
You hereby agree that we may communicate with you electronically or by fax and that any signatures send electronically or by fax have full legal force and effect.
This agreement is governed by, and must be constructed and enforced in accordance with, the laws of the State of Oregon, excluding principles of conflicts of laws. For every dispute regarding this agreement: (a) the prevailing party is entitled to its costs, expenses, and reasonable attorney fees (whether incurred at trial, on appeal, or otherwise) incurred in resolving or settling the dispute. In addition to all other damages or awards to which the party may be entitled; (b) each party consents to the jurisdiction of the courts of the State of Oregon and agrees that those courts have personal jurisdiction over each party; and (c) venue must be in Multnomah County.
